In females, the bacteria frequently ascend past the cervix into the sterile upper reproductive tract, infecting the uterus and fallopian tubes. This triggers massive intra-abdominal inflammation, causing profound pelvic tenderness and severe structural scarring. It is a critical warning sign of ascending pelvic inflammatory disease.
